Biotin and Blood Tests: Why a Beauty Gummy Can Skew Your Lab Results

The most important biotin fact is not on most labels: at the doses in hair and nail gummies, it can change the numbers your doctor relies on.

The chemistry in one paragraph

Many automated blood tests, called immunoassays, use biotin as a handle. An antibody carrying biotin grabs the substance being measured, and a surface coated with streptavidin grabs the biotin. When the blood sample itself contains a lot of free biotin, that free biotin occupies the streptavidin and the signal goes wrong. In sandwich tests, such as troponin and TSH, the result reads falsely low; in competitive tests, such as free T4, it reads falsely high.[1]

The biotin line on the label

What has actually happened

In a controlled JAMA study, healthy adults took 10 mg of biotin a day for a week; several commonly used hormone and non-hormone tests then gave clinically misleading results.[1] Case reports describe patients whose thyroid panels looked exactly like Graves' disease until biotin was identified as the cause.[2] The FDA has warned that biotin can cause falsely low troponin results, the test used to diagnose heart attacks, and publishes a list of affected assays.[4] A Mayo Clinic study found biotin supplement use common in outpatients and measured interfering levels in some emergency patients.[3]

Tests most often mentioned

TestTypical direction of errorWhy it matters
TroponinFalsely lowCan hide a heart attack
TSHFalsely lowMimics an overactive thyroid
Free T4 and T3Falsely highMimics an overactive thyroid
Some fertility and steroid hormonesEitherCan mislead treatment decisions
Some vitamin D and other assaysEitherDepends on the platform

Not every lab uses affected platforms, and manufacturers have redesigned some tests. Only your lab knows which of its tests are vulnerable.

What to do

  1. Tell the clinician ordering the test that you take biotin, and how much. A 5,000 mcg gummy is a high dose in this context.[5]
  2. Ask the lab how long to pause before routine tests; the answer depends on dose and test.
  3. In an emergency, say it out loud at triage. Do not wait to be asked.
  4. If a result surprises your doctor, mention biotin before any treatment change.

Where Gelagen fits

Gelagen supplies 5,000 mcg of biotin per serving, and its label carries no laboratory warning. Our safety file covers this and the product's other cautions.

Quick questions on this topic

Which blood tests does biotin affect?

Immunoassays that use biotin and streptavidin, including some troponin, TSH, free T4 and T3, and certain hormone tests. Each lab knows which of its own platforms are affected.

Do I need to stop biotin before a blood test?

Ask the lab or the clinician ordering the test. The pause depends on your dose and on the specific test; never guess, and always disclose it.

Is 5,000 mcg considered a high dose?

Yes, in this context. It is about 167 times the 30 mcg adequate intake and in the range discussed in the lab-interference literature.
